Your first visit you must have
your parent present to sign the liability waiver. The only alternative is
to print the waiver and sign it in front of a . The be present on the submitted waiver form for it to be accepted.
There will be NO exceptions to this rule. Please be sure to print waiver
on 8½" x 11" (letter size) paper. There can be NO alterations
to the form whatsoever or form will be considered null and void.
